Your first appointment

What to expect at your first visit

Before you arrive

Please bring a photo ID, current insurance card, an up-to-date medication and supplement list, and any recent laboratory or imaging records you have available. Complete the New Patient Intake Form and HIPAA acknowledgment before your visit when possible.

Arriving about 15 minutes early gives our team time to review your registration information.

During your consultation

Your nephrologist will review your medical history, medications, kidney function and reason for referral. Depending on your individual situation, additional blood or urine testing, kidney imaging, or other evaluation may be recommended after the consultation.

Bring any questions you would like to discuss—we want you to understand your kidney health and treatment plan.
